Instructions:

  1. Download the Form
    Please download the required fillable PDF application form. You will need to fill it out on your computer or device before uploading it.

    You may sign and initial electronically with a digital image of your signature. There are several programs and apps that support this. Photos of the signed and initialed pages, merged into one PDF, are also acceptable.

  1. Complete the Application
    Open the downloaded PDF form on your device and complete all required fields. Ensure you provide accurate information for both the Student Name and Parent/Guardian Name. Please complete the application in its entirety. Incomplete applications will not be processed.

  2. Save the Completed PDF Form
    After completing the form, save the PDF to your device. Be sure to save a copy for your own records as well.

  3. Upload the Completed Form
    Return to this page and fill out the fields below:

    • Enter the Student Name.
    • Enter the Parent/Guardian Name.
    • Use the Upload Completed Form field to upload the filled-out PDF.
  4. Submit Application
    After uploading the completed form, click the Submit button to send the form. You will receive a confirmation email, and the submitted form will be reviewed once we receive your application fee.

  5. Submit the $75 Application Fee
    Please note that a nonrefundable $75 application fee is required for your submission to be processed. We currently only accept payment by cash or check. After submitting your form, please mail a check to the address below:

    Be sure to include the student's name on the check to ensure proper processing.
